Lesions: The fact that one lesion enhanced, presumably on mri, is what makes it more suspicious for ms. There are times when the diagnosis is made purely on mri. Your enhancing lesion is pretty small. You will need a follow up MRI in 3-6 months to see if new spots develop. People with headaches are very likely to have abnormal spots on MRI that are benign, but don't usually enhance.

Sorry, but: The old lesion and a new enhancing lesion raise significant concerns that you possess the initial characteristics of MS, so called "Radiologically Isolated Syndrome". So, stay ahead of the process, get an MRI of neck, and a lumbar puncture, with some blood tests to rule out other causes. Have a neurology consult ASAP.
