Likely a virus: Up to 2/3 of throat infections are due to viral illnesses & will eventually heal without antibiotics.Use of throat lossenges & sprays help.An effort to should be made to maintain proper fluid intake in spite of discomfort as it will shorten the illness in most cases.

sudden illness, large submandibular lymp nodes, exudate (white spot) on throat (tonsilar pilars), fever >100.5, age under 15 years old are highly suggestive of strep. If rapid strep is negative, and suspicion is high based on above criteria, then we tend to treat and do cultures (take a few days for results)
