A FEW COMMENTS: Seratonin syndrome in my experience is quite rare, and seems to be due to higher doses of meds which affect neurotransmitters. There is no real magic number, and the process is influenced by biological variation and metabolism. But, clearly, if you slowly drop the paroxetine dosage, and initiate the duloxetine at a very low dosage, you can taper off one, and slowly advance the other without risk
