Highly unlikely: TB lymphadenopathy is initiated when macrophages carry the bacillus from a pulmonary alveolar focus to the lymph node which then enlarges, suppurates and ruptures into an adjacent blood vessel. So it is virtually impossible to have pulmonary nodes involved without having pulmonary infection first.
