Maybe, maybe not.: Only an x-ray can show for sure whether or not a permanent tooth is still under your baby tooth. At the age of 21 it is most likely that you are congenitally missing your permanent tooth. This is not that unusual and often that baby tooth can stay there for many years into adulthood. You should see a dentist to evaluate the integrity, position and functionality of that tooth. See your dentist.

Baby tooth: Either there is no permanent tooth replacement or the underlying permanent tooth is impacted. Removing baby tooth without making arrangements for replacement can result in posterior bite collapse. Your Dentist can tell with an x-ray what's doing in that area.
