Secondary/tertiary: T4 if from the thyroid. Tsh is from the pituitary. Tsh is released as response to trh from the hypothalamus. T3 (liothyronine) is conversion of T4 in the tissues. T4 and t3 (liothyronine) gives back signal to the brain. T4 decreases if there is no TSH to the thyroid. It seems that is the problem. It could be lack of trh but unless it is from a tumor it is inconsequential now. Give thyroid hormone and follow the levels, not tsh.

Don't base on TSH!: Most doctors base diagnosis and treatment of hypothyroidism almost entirely on TSH but TSH is surprisingly inaccurate. I base my assessment primarily on signs and symptoms and also free t3 (liothyronine). You may need more thyroid even with a TSH that low and may also benefit from adding t3 (liothyronine).
