OTC products: It is unfortunate that you have had a problem with your current dentist. Chances are very good he/she has discarded the models.. If we kept every model of every patient we would need a warehouse! store bought guards do not fit well and will not be comfortable. Insurance or not, prioritize your funds and find a new dentist who you like to help you with your bite problems.

Bite Guards: Bite guards vary in materials used, design, and in-office vs. Lab fabricated. The costs also vary from office to office. I personally use a wide range of materials, labs, and designs. Some appliances for tmjoint issues require a lot of chair time customizing the bite. I would guess fees range from $200 to $1000. Discuss your particular situation with a dentist who can personally examine you.

Depends: It depends on what type of guard you're getting. If you are wanting the top of the line and done with muscles/trajectory/physiologic rest position, it may be several thousands, or performance mouthguards for sports such as underarmour or thermosplints, hard splints, nti, ... There are just so many options.. Ask your dentist for what purpose you are desiring... Just bruxism/ attrition or TMJ ?

Also costs vary: There are many types of mouthpieces for tmj/tmd- upper/lower/ soft plastic/hard plastic/ etc. Also, the symptoms that you have dictate the type. So determining a cost is impossible without reevaluating.
