No: There's no way you can know he has strep throat. Doctors can't either without a test. Most sore throats are caused by viruses and shouldn't be treated with antibiotics. Also, antibiotics have very little effect on the symptoms of strep throat. They're given only to prevent rare complications.

Azithromycin dose: Yes it is ok to crush a tablet of Azithromycin and give 250mg dose.
