: If indeed it's blood under the big toe, one might have stumped or traumatized it unknowingly. But Sometimes Fungal toe infection and other ailments can appear like blood under the big toe. Good idea to see Foot Doctor/Podiatrist to be sure. and have it well taken care of.

A clean up procedure: Cheilectomies are "Clean-up" procedures to help relieve some of the signs and symptoms of an arthritic joint, while still preserving the joint.   Not quite sure why the toe is out of alignment.   The alternative to a cheilectomy is an implant.  These are much improved over the  past few years and are quite similar to a knee or hip titanium appliance.

Generally local prob: if the numbness is isolated to big toe often it is local problem such as shoes or pressure from shoe wear. sometimes referred from a higher nerve source.

Maybe not so easy.: More to the point, before trying to self diagnose, have it examined and eliminate all doubt. Your symptoms seem to describe a gouty condition, and could very well be, but controlling diet may be only part of the solution. Your doctor can supply all the necessary information, as well as instruction, to help you deal with this.

The information : Provided is very limited and the reasons can be plentiful including and not limited to bursitis, gout, arthritis, tendonitits, neuritis, even nerve impingement in the back. It would be helpful if more information is provided like what causes pain to increase or what time of day, what you wear on your feet etc does it happen at rest, while walking etc, the more you provide, the easier to help you.

Why sore ?: We need more information on the irritation to determine if this is an ingrown toenail/infection/fracture/metatarsalgia/neuroma..?  Please send more information. 
In the meantime, see if you can get an orthotic or carved sole that will carry your weight of the back 3/4 of the sole and totally off weight the painful area.

: It sounds like you are developing pressure points on the big toes. This sounds like a protective callus from shoe irritation from the shoe or a weight bearing pressure point.  you change shoes this area could resolve itself. If this continues and painful see a podiatrist to help with shoe choices
