Dr dx gout based on symptoms of sudden bad pain in big toe area randomly. why would attack randomly occur even when diet is consistent daily ?

Not random: If you do have gout, the attacks are not really random, but also not always predictable. What happens is uric acid crystallizes in the soft tissues during an attack and this then can cause a rather severe inflmatory response from the body.
