Orthostasis ?: If this happens as a result of change in position, such as standing up or sitting up from a lying down position, you need to see a Cardiologist. If it is a stress response, such as when you feel anxious, you need to explore your feelings with a psychologist to reduce stressful activity in your life

how long does it take for a heart rythm to go back to normal after a 30 minutes fast walk/jogging? is it normal to still be in the 90 after 45min of finishing exercise?

Hr after exercise: It depends how much you are in shape. Is it the first day of exercise after being not so active for a long time .and also what is your bSeline HR before the exercise . Usually no it does not take that long for hr to return to normal -10-15 min of resting should make you very comfortable ,  drinking  fluids also help . If you have any concerns or not feel right after the exercise , see your PCP and

today i felt a pain in the upper center back and center of chest suddenly and it lasted only seconds. no other symptoms, now i feel normal but sometimes i feel like a little pain in my back, again for seconds and it go away. can it be heart related?

: The stomach entry is just below/in back of the heart. This pain often represents the effect of stomach acid back-washing at this point. Caffeine/nicotine/acidic foods aggravate this as does laying down after meals. TUMS can sometimes give brief relief. The back issue could be separate.  Sorting this out would require a follow-up with your doc.

: More likely sinus arrhythmia, normal variant.    See your PCP who will perform physical exam and  may run a screening EKG.

Not uncommon: There are many reasons people have the feeling of rapid heart beats (tachycardia), some have to do with the heart and some have to do with the mind. Did you have the palpitations while you had the holter monitor on; if it  "catch it", i would suggest an event monitor instead. Most cases are benign and self-limited, rarely requiring treatment. Did you have your thyroid checked?

PVC: The heart has a pacemaker (PM) or internal clock in the upper chambers (atria) that sends a signal to the lower  chambers (ventricles) which is delayed so that the ventricles contract after the atria. Premature ventricular contractions (PVCs) can occur spontaneously or be triggered by adrenaline or drugs and are always followed by a compensatory pause to return the heart to its previous rhythm.

WPW: Yes, you should be able to.  Wolfe Parkinson White is a re-entry tachycardia.  EP Cardiologists abate the re-entry path to restore normal sinus rythem.

Not serious: You are likely describing premature heart beats which are not a sign of heart disease in an otherwise healthy patient. The pair of fast heart beats consist of a regular beat followed immediately by a second premature heart beat. These premature atrial(PAC) or ventricular(PVC) contractions are not uncommon. Anxiety, fatigue and caffeine can cause an increase in these premature beats.

i have had heart issues and other heart related symptoms for years now. my echo, stress test, and ekg results all came back normal. however i am in pretty concerning pain almost every day and my heart rate goes up to 240 sometimes. what do i do now?

Monitor: Have you had heart monitoring as an outpatient? This should demonstrate the nature of the tachycardia you experience. Also you would write in the diary when you experience chest pain or palpitations. A heart rate of 240, if accurate, would be caused by a significant cardiac dysrhythmia. A maximal treadmill stress test would rule out coronary blockage or spasm. You must have a cardiologist.
