i have pcos on the low end and i am on metformin trying to get pregnant again. dr says i should try fertility treatment. is it necessary?

PCOS and pregnancy: Pcos often causes infertility, however with vigorous proper treatment many women can get pregnant.  Everyone is different.  Pcos is often associated with being overweight, excess androgen secretion, hyperinsulinism, metabolic syndrome and hormone abnormalities.  It is advisable to see specialists with a major interest in pcos.  Diet, weight control, ample exercise and medications are indicated.

trying to get pregnant with pcos. my husband and i have been trying to conceive for two years now and have gone through treatments from metformin to clomid (clomiphene). is there anything else i can do?

IUI or IVF: Based on your age you should consider a more aggressive approach to fertility treatment since Clomid (clomiphene) alone failed to lead to conception. A limited number of intrauterine insemination (iui) cycles may be an option. However, in vitro fertilization (ivf) may be a better option.

how does fertility treatment for lo-cah differ from fertility treatment for pcos?  i'm young, normal-weight, ovulate rarely, and am otherwise healthy.

Yes: Even though the 2 disorders have the same symptoms, their causes are different. Most docs will treat cah with steroids (to suppress the adrenal output) and pcos with with some combo of Clomid (clomiphene) and/or metformin (to treat Insulin resistance and induce ovulation).

Safe; sometimes used: Progesterone or artificial versions (progestins) can be safely used to induce a period for women with pcos who have very few periods (less than 4 a year) or no periods. It does not improve your chances of conceiving because it induces bleeding, not ovulation (egg release). Progesterone can also be given for low Progesterone levels in the 2nd half of a cycle, or to support an early pregnancy.

MAYBE.: IF MENSTRUAL CYCLES RESUMES SPONTANEOUSLY YOU CAN HOLD OFF METFORMIN..ALTERNATIVELY IF PERIODS ARE IRREGULAR WOULD RESUME METFORMIN. BUT   BEWARE  OF  POSSIBILTY   OF PREGNANCY  IF NOT ON SOME FORM  OF CONTRACEPTION.

: These conditions vary in intensity with each case and many go on to have children. The average wait for a desired pregnancy without these factors is 6 months with a range of weeks to a year. If you double that as an expectation I would likely do a fertility center consult after 2 years without success. Both parents are studies, since both must contribute to the success.

Women can use the: same type of pregnancy test whether they have PCOS or not. Some women with PCOS may become pregnant without fertility treatments. Ask your physician about your specifics.

Lose weight...: If you are not ovulating because of PCOS, Metformin alone may not be enough.  You may need a fertility medication such as Clomid (clomiphene) to force your ovaries to ovulate.  If you don't want to use a fertility medication, you may need to lose weight.  Losing as little as 10% of your weight can help you to ovulate.  Best wishes!

Weight loss?: If you are overweight, losing weight will help your ovulation. Also, if your periods are irregular or absent the drug, clomiphene, can be more helpful than even metformin (for getting pregnant).

PCOS is a diagnosis of exclusion, meaning since there is no specific, definitive test for it, other conditions (that are treated differently) may need to be excluded before PCOS is the settled upon diagnosis. These most commonly are congenital adrenal hyperplasia and Cushing's syndrome. The most common treatments for PCOS are metformin, BCPs, and spironolactone, either alone or in combination
