when i wake up in the morning i have no signs of my tinnitus. i get to moving and in about 15 minutes it comes back and it sounds like a pulsating hissing sound. any ideas on what could cause as ent determines no hearing loss?

Sounds OK: If you have conductive hearing loss on the right side only and a normal-appearing eardrum, the most likely cause of  hearing loss is otosclerosis.  This is fixation of the stapes bone in the middle ear.  This can't be seen on scans; the most reliable way to confirm in middle ear exploration. Some ear surgeons will perform a CT to assess for other causes, but most do not.

Perforated TM: I am not sure what is your question. If it is a perforated drum, generally it should be surgically repaired unless there is a particular problem, such as severe eustachian tube dysfunction. Even then, some solutions are possible. The ear usually should be kept dry. You may need a second opinion.

Otologist: If you've seen an ENT he has done tests and given your a diagnosis/treatment. If you want another opinion, I'd recommend an otologist. A ENT expert with a additional fellowship in ears.

Ecog test: There is more to the ecog test results than what you report. The audiologist reports all the findings to your Otolaryngologist and he/she then gives you the results. Certain criteria have to be met before diagnosing meniere's disease.

It happens/rare: Mastoiditis is rare today in children or adults that have access to medical care. When there is a recurrent middle ear issue there is often mild hearing loss due to the muck that accumulates in the middle ear and interferes with the ear bones ability to amplify hearing.Over time, it should improve &amp; regular monitoring is your best way to assure that it does.

Tinnitus: Ringing in the ear or tinnitus. Looks like you have already seen an ENT doctor, he/ she should have told you the remedy for this. If no correctable cause can be found, ENT or audiologist may help you to try for a masking device. Seek help.

ent: would you prescribe steroids (oral or intratympanic) for sudden worsening tinnitus 12 days ago with no hearing loss? saw an ent that recommended i observe and not treat. afraid it may come permanent. should i seek a second opinion?

: Tinnitus itself is a difficult process to treat, and most often occurs in context of hearing loss in older folks.  Can be a side effect of medications, recreational drugs, loud music--especially via head phones and rock concerts, and work related noise exposure.  Not clear if steroids really ever help.  Yes, get your second opinion, and consider evaluation for Meniere's?
