At what age did your wife receive the vaccination? She might expose to the virus before the vaccination and if she was not, the vaccination is not 100% effective.

Most likely she had a dormant HPV infection from the distant past, probably before she was vaccinated, and it reactivated. This does not imply that either you has had new sex partners. This is neither her fault nor yours, just. Discuss with her gynecologist. Good luck.
