Phimosis: You are suffering from phimosis. If this is troubling you, you should make an appointment to see a urologist. Conservative therapy would be topical steroids. Surgical option is circumcision.

Scar on Foreskin: See your urologist for scar revision. Traditional corrective surgery is done via circumcision. If you are not concerned about cosmetics, a simpler procedure called a dorsal slit can be utilized. In this operation the foreskin is left intact but an incision is made through part of the prepuce so as to release the foreskin totally in the midline almost to the rim of the glans penis.
