Go to hospital!: Coughing and vomiting up blood are symptoms that suggest a serious issue with either the lungs or stomach. If persistent, it can lead to worsening anemia (low blood count) and be potential life-threatening. It is safer to get checked out as soon as possible in the emergency room; don't wait until it becomes more serious!

This is very hard: Without knowing much, much more no one can tell you (or him) the right thing to do. Clearly you would go to the hospital. If you've already talked with him about his views on medical issues and dying, perhaps you know his views. Are there other family members involved? Does he have a most trusted other? Is he otherwise well? So many questions. Do what you can to get his compliance. I feel for you.
