Many reasons : Most common is diverticulitis or cancer. Cancer can b easily diagnosed with colonoscopy and direct vaginal exam. Ur gyn doc and general surgeon can give you more info.

Usually traumatic: The connection may be related to an infection of the colon (diverticulitis) or after vaginal delivery. Usually will require surgical repair. Not usually related to cancer.
