Poor inspiration: Probably related to a poor inspiratory effort when the radiograph was taken. Often times if people are in pain or are obese they cannot take a big breath (which would expand the lungs and give a better look to the radiologist) so what we see are low lung volumes and small areas of collapsed lung - all told it is usually not a sign of anything. Sometimes atelectasis can cause a fever.
