Mildly underactive: Would help to know your T4 but with a TSH of 20 you probably have a mildly underactive thyroid - many causes but the most common is autoimmune thyroiditis. You may not have any symptoms of low thyroid with TSH of 20 but you should see an endocrinologist if you have not done so already.
