Hypothyroidism: high TSH >4.0 is often times associated with highish prolactin as in your case. With high cortisol, assuming the labs were done in early am and fasting, may suggest adrenal stress with superimposed sluggish thyroid. Suggest dropping off gluten,dairy and legumes for a month then rechecking (ask for thyroid antibodies to be ordered if not checked before) good luck

What time of day?: Hi. they may be normal. The TSH could be a sign of early, subclinical hypothyroidism (getting TPO and thyroglobulin antibodies and free T4 may help the assessment). Due to diurnal variation, if the blood was drawn around 8 AM, the cortisol would be normal; very abnormal if drawn late at night. The prolactin, depending on the lab's normal range may be normal or slightly elevated.
