Rheumatic heart dis.: Without knowing more, my answer must be limited. But if you do have rheumatic valvular disease, you should be checked periodically. At age 24, if evidence of damage is mild, you probably don't need yearly checks, but you should be checked to be sure the problem isn't damaging your heart further.

RF can cause: Permanent damage to the heart, including damaged heart valve and heart failure. If you have a heart condition cause by rf you should be follow up yearly by your cardiologist. The follow could be longer if recommended by cardiologist.

Echo every 5 years: Generally, rheumatic fever causes a slow, progressive degeneration of heart valves over many years. An occasional echo lets you know if anything is developing. They are relatively low cost tests with no pain, needles or radiation exposure. I recommend an echo every five years to anyone with a history of rheumatic fever.
