Low blood sugar?: Usually causes shaking, sweating, increased heart rate and changes in vision. If you are experiencing nausea and chest pressure, seek attention (urgently) to rule out cardiac causes.

Well, I would: Say it's unlikely ibs. The immediate concern is cardiovascular so i would see your doc. Next reflux, may get relief from these meds, next anxiety which exacerbates everything. Make sure heart problems are ruled out as soon as possible, good luck.
