Cataract & diabetes: Cataracts typically develop with advancing age but can also be associated with diabetes. This is especially true if a patient doesn't control his blood sugars well. In a 46 y/o patient, a significant cataract could be caused by either of the above, or possibly even other causes. The decision about having surgery is based on the severity of the cataract and the patient's symptoms.

Yes: Diabetes can cause a specific type of cataract to develop called a posterior subcapsular cataract (psc). 46 is young to have the typical age-related cataracts, so a psc is likely. Besides diabetes, trauma, steroid use, and recurrent intraocular inflammation can also cause cataracts to form at an earlier age.
