Need more details: There is a wide spectrum of appearances of echogenic (bright) foci, varying in number, distribution &location, in an otherwise normal ovary. Specular reflection from walls of tiny unresolved benign cysts can appear as bright foci without shadowing. Many things appear echogenic on ultrasound including calcifications, fat, bone, gas...Echogenicity is a relative term compared to background parenchyma.
