Depends on you: Most of the time under general anesthesia as it is patients choice, how ever if patient wants could be done awake , spinal regional , anesthesia.

Bunion surgery: I do not recommend being awake you can either have general or IV sedation with local block.

Awake: I don't recommend it as the sounds in the operating room can be upsetting to some patients resulting in involuntary or conscious movement. I feel it is best to be in a twilight sleep with local anesthesia.
