Depends: Some small amounts of fluid in the female pelvis is physiologic. Larger amounts can be associated with ruptured ovarian cyst. Pelvic inflammatory disease can produce pelvic fluid. In the case of acute appendicitis free fluid can occur as well. Inflammatory bowel disease is another cause of fluid production.
