Could be: Usually blood in the eye or hyphema is caused by severe blunt trauma, so it is important to follow up and be certain that after the blood clears away, the other underlying structures in the eye are healing up properly. There is an increased risk of glaucoma, cataracts, and retinal detachments associated with having a hyphema, so be sure to keep the child going for regular follow up.

Was it a hyphema: Usually temporary: depends on vision (blocked vision for a long time in a young child could necessitate washout), eye pressure (if high for a certain amount of time, could need ac washout and/or glaucoma surgery), any coexisting injuries (how was the retina), otherwise if it's a small amount of blood, it may clear with steroids and reduced activities, upright position, avoiding blood thinners, .

Eye problems: A small spot of blood can appear in eyes due to excess coughing or at time of birth . Follow up to see it regress. Should not cause a problem .
