I have hallux limitus. one doc recommends midfoot fusion to lower the first metatarsal more than a modified youngswick can. is this a good idea?

One would need to : Evaluate your x-rays and foot to give you an informed opinoin. Usually, for hallux limitis "head work" like a youngswick should work out fine. If in doubt, seek another opinion.

Hallux limitus: Good question.... Without seeing you and seeing your xrays/records it is hard to give an accurate opinion. However, the procedure that is proposed is complex and you need to be handled properly. Usually a simple hallux limitus procedure is indicated.

LAMBRINUDI: Without an exam, I ask myself if an isolated 1st metatarsal/1st cuneiform fusion is better than a whole midfoot fusion. But, if you really need plantarflexion, the Lambrinudi plantarflexory osteotomy may work. It avoids a fusion. It's performed at the 1st metatarsal base. Good luck. Dr. Latva
