Heart attack: You can have a heart attack and have normal vital signs. There are many causes of chest pain that aren't the heart. If you smoke and have high blood pressure you should be doing better taking care of yourself.

Yes: Pulse can be quite variable bp can be up or down ( usually up with pain) temperature can vary but only a bit. Oxygen saturation can vary.
