No: Being tongue tied is not a painful condition. Would think more on the line of gum disease/gingivitis or canker type sores in his mouth. This time of year hand/foot/mouth disease is also a consideration, but he is a little bit on the old side for that (we usually see that in the 3 and under age group, but not impossible for a 4 y/o to have it). Do good mouth hygeine, and see doc if doesn't resolve.

Possibly: The problem may involve the attachment of the tongue but could also be many other things including tooth issues as well as blocked salivary glands. Please go to your dentist.

NWTK without: Honestly, there is no way to know without actually examining your son. Merely taking some guesses serves no one. I highly recommend that you take him to a pedodontist (or a general dentist that treats children at this age). It's unlikely the ankyloglossia has anything to do with the discomfort. He may just have a canker sore or mild abrasion from food or the toothbrush, but still needs to be seen.
