Could be: Viral illnesses are common in childhood.A whitish tongue is often more a reflection of not eating than anything else. If viral it can involve many vague symptoms over several areas of the body.As long as the kid keeps up with his fluid needs you can monitor the issue. If the pattern becomes more worrisome, he can always get in & be evaluated.

Needs ENT referral: We have been treating bad breath for over 27 years. Not many parents complain on kids breath... But we have seen few cases similar to what you describe, and - surprisingly enough- the reason was discovered by ENT - kids sometimes push in their noses small objects, that do not cause pain or discomphort, but prevent normal discharge from the nose, so after some time it causes very strong smell..
