Heart attack: Ignoring a heart attack can be fatal, depending on the severity. Sometimes people have had past heart attacks that they didn't realize at the time. Resulting damage to heart muscle can show up on later ekg's. If symptoms are happening in the present, cardiac enzymes will be elevated also. At age 20, there are more likely causes for chest pain than heart attack, though.

Need to be tested: Lot of small heart attacks can go unnoticed and you can lead a normal life. Overall heart function is important. If you are a diabetic you can have a big heart attack without knowing. A lot of things may feel like a heart attach too. Simple 12 lead ECG may be enough to give some information.
