Not sign distress: His hives are not likely a sign of distress, but a sign that his immune system has recognized a foreigner in his body and is trying to mount a response to the new intruder. This is actually a good sign his body is responding. Treat hives with diphenhydramine and monitor closely for any signs of respiratory issues or of swelling in/around mouth and throat.

COVID 19 infection: It is important that his diabetes remain under good control. Skin symptoms can occur with COVID 19. Get urgent care for difficulty breathing, blue lips or face, persistent significant chest pain/ pressure or new onset confusion.
