Notnecessarily at all: Size of fibroids not important per se. Location more important. Fibroids along uterine lining (submucosal) often cause heavy bleeding (particularly those 4.5cm). Fibroids on outer part of uterus (subserosal) often cause bulk-related symptoms (urinary frequency, pelvic pain) by pressing on adjacent structures. In between those (intramural) may or may not cause symptoms. If so,see fibroid specialist
