CAD: Whether a patient with CAD and low ef can be helped by CABG is dictated by their coronary anatomy, the reversibility of ischemia present and their total body status. You need to discuss with your cardiologist who knows you best.

Not usually: CABG is usually done earlier, but good question and occasionally someone is suitable and improves lvef.

First step: is making sure all living muscle has as good a blood flow as you can get - test the heart for scar and viable tissue. then depending upon the coronary anatomy, "revascularization" options (CABG, stents) may exist and if so, this may greatly improve outcomes.
