She needs to see a: Doctor. It is not feasible to provide a meaningful opinion without more history, physical examination and may be some tests. It would not be a good idea to delay seeking care.

See details: Strep is generally not a urine pathogen. A high ANA needs to evaluated in context with a history, physical and other lab studies. See a rheumatologist.
