Many reasons: Is she using her drops as prescribed? Is she using the proper technique to get them into her eyes and keep them there until they can be absorbed? Are the drops stored properly? A few need to be refrigerated. If you are sure she is using them properly, then it is possible she needs another combination of medications to control her pressure. Go over all of this with her and her ophthalmologist.
