It is usually hard: To address this type of disc herniation through a posterior approach unless it is a very lateral or to side hnp and causing only arm pain. Otherwise, the recommended surgery is an anterior discectomy and now with a laminectomy present a fusion has to be done. If a laminectomy had not be done in the first place, an anterior discectomy with a total disc replacement could have been an option.

Neurosugery : In situations where there was a laminectomy and continued pain with new MRI findings you need to see a neurosurgeon. The doctor will confirm by exam and maybe nerve testing that your symptoms correlate to new MRI findings and then may recommend fusion.

See specialist : It sounds like you need to see a spine specialist to undergo an evaluation. If conservative care fails, i would suggest an anterior approach next time. Check out spine-health.Com. Good luck.
