Fever is Uncommon: A fever has usually broken within 48 hours of being on antibiotics for an ear infection. If the fever continues, it either means that the antibiotic is not covering the bacteria causing the ear infection (in which case you need to switch to a different one), or that there is a viral illness associated with the infection.

Inappropriate antibi: if it has been 48 hrs and his fever is 104 he needs to be seen again for re-evaluation and possible change of antibiotics
