Confusing question: It is unclear from your question how long this has been going on, how the label "strep" was made & what you mean by 24hrs on high dose. Treatment of ear infections in kids with amoxicillin is based on 90mg/kg for the duration of treatment, not 24hrs. Strep would respond to this or lesser amount of abx. Persisting fever after 48hr may indicate drug failure or co-existing viral infection.
