TMJ: Assuming your symptoms are structural due to the temparomandibular joint (tmj), you should probably have further consultation with an oral surgeon an get a ct scan of the joint. You are quite young to have such severe symptoms. Often it is due to night grinding of the teeth, but it sounds like you've already tried the guard.

Lock jaw: You probaly have a dislocated disk in your joint and need to be evaluated by a dentist that specializes in TMJ disorders or and oral and maxillofacial surgeon. You may need further treatment with a properly made bite guard that you wear full time.

TMD Specialist: There are not many dentists with sufficient experience and training to handle cases such as yours. It's a complex issue that requires a substantial evaluation and multiple methods of treatment. Most dentists merely make a ng and suggest palliative measures. You will have to do some research to find someone adequately trained to handle tmd\tmj issues.
