Crohn's : Obtaining tissue and looking under the microscope, generally from a colonoscopy or upper endoscopy. Sometimes the way the small intestine looks on x-ray can suggest the diagnosis.

History, exam,labs: A combination of a careful history, a thorough physical exam, blood tests, maybe some imaging, and definitely an upper and lower endoscopy. These are all used to help determine whether you have Crohn's. There is no "one best test". All of them are used in combination to come to diagnosis.
