Many Options: First most cities have clinics or health depts. That provide free or low-cost medical services or income-based payments. Many accept care credit or other similar financing. These companies lend money for medical purposes & accept a higher risk since most people pay for medical care before they pay other bills. Doctors are not banks & do their best work when they are not being bankers!

Inability to pay: Some doctors in private practice allow for a small % of pro bono work. This is rare, not because they don't want to, but because they have a duty to treat patients in the same fashion, with rare acceptations. Expensive part are the meds. Most pharm companies have a patient assistant program & they actually do work. Maybe "dr. Obama" will provide a better answer.
