Can doctors have a good idea if you have crohn's during a colonoscopy based on features and characteristics?

Usually, yes -- by visual appearance confirmed by biopsy. Other lab tests like fecal calprotectin and blood inflammatory markers (sedimentation rate, C-reactive protein) also are helpful.
