Several, but rare. : Most prostate cancers in this country are detected by psa screening, and have no initial signs or symptoms. Beyond this group, the first sign can be a nodule or lump felt on the prostate during a digital rectal exam (dre). More advanced cases present with symptoms, such a difficult urination, bone pain, and neurologic deficits, but his is much more rare in a contemporary screened population.

It varies: Prostate cancer is often silent and is typically picked up on a screening blood test called psa. Sometimes patients have urinary symptoms which may include frequent urination or blood in the urine. Some patients have back or pelvic pain which may be indicative of more advanced cancer.
