Hemorrhoids: likely not. The causes include hemorrhoids , anal fissure, which are common being conditions. Anal fissure causes pain after defecation. The possibility of a neoplasm or cancer in the anus or rectum needs to be entertained.to start , an ano rectal exam is needed. Followed by an endoscopic evaluation. Of course being anemic , goes with cancer. Angiodysplasia or fragile vessels may be causative too
