A good discussion with his primary care doc or nephrologist as to what the goals are from his or her point with 2 main areas longevity/quality of life as they will take away from each other. That should give peace of mind to all involved and provide for better planning and a smooth transition to hospice for when and if needed.

His nephrologist would be the person to give him/her the specifics of a diet as they know his condition best. You or the family should try to convince him , if possible and eligible for dialysis or transplant. Good luck
