Transvaginal ultrasound is often the initial study for evaluating for abnormal bleeding and endometrial cancer. Studies have used a cutoff measurement of 5mm in post-menopausal women. However, for premenopausal women, an optimal cutoff has not been defined, and some recommend 16mm or less. An endometrial biopsy may be warranted.
