I do not understand the term irresponsible sinus tachycardia. You should get a cardiac clearance by a cardiologist if dentists refuse to treat you. Good luck.

There are risks imposed by your tachycardia. I assume that you're under the care of a specialist Cardiologist. With your history you should be seeing a specialist Oral Surgeon for extractions of any sort. The Oral Surgeon should communicate with your Cardiologist, and vice-versa. It may be prudent to do the procedure in a hospital setting.
