Tachycardia: Seems that you have innapropriate sinus tachycardia. You should have a cardiologist look into that.

Without more: Information it's difficult to arrive to a recommendation or a conclusion. It could be a heart arrhythmia, could be simply you are out of conditioning, but also could be a condition like hyperthyroidism. An ekg and a good history and physical needed asap.
